pop_health_graphicAs we’ve discussed before, Population Health, accountable care or the pursuit of a sustainable healthcare economy that works for all Americans and advances the triple aim requires data and the Population Health Alliance Framework includes identify, Assess, Stratify, Engage, Intervene and measure. In the Cerner lingo this is framed as: ‘Know, Engage and Manage’.

Cerner has a number of tools in their HealtheIntent platform that provide a backbone for Population Health Management. We’ll discuss with Brad, Cerner’s thoughts on population health management, what services they provide and where Cerner sees the future of Population Health.
